Indiana is using CFTMs as one of its foundations for Practice Reform. A CFTM is a meeting where key people come together and collaborate, facilitated by a DCS Family Case Manager, to build a system of support and accountability for a family that has a child or children in the system. The meeting is driven by the wisdom and expertise of families with outside support the family has chosen, such as extended family members, friends, neighbors, and community members. Formal resources like mental health providers and schools may also participate in the process to maximize stability and permanency for the child.
