If you have had a legal name change, you must submit a copy of your legal name change document such as a marriage certificate, divorce decree, or legal order. Please include your license number with your request. Social Security cards with a copy of your current Legal ID may also be accepted.
You may login to your account at mylicense.in.gov/eGov/ML1PLA.html to upload your legal name change document by using the ‘Upload Additional Documentation’ action item. If you have not yet linked your record you will not see the available action items. Select ‘Don’t see your license? Click here to search for it.’ And enter two search criteria to link your license.
Once your name has been updated, you may log back into your account to print a free copy of your license. Should you encounter any issues, please contact our office at 317-232-2960.
