A package of OSHA forms for recording work-related injuries and illnesses (includes OSHA 300 Log, OSHA 300A Summary, 301 Report, and instructions), in a PDF format, may be downloaded from http://www.osha.gov/recordkeeping/new-osha300form1-1-04.pdf.
