An INSPECT report summarizes a patient's controlled substance prescription history. Each time ephedrine, pseudoephedrine or a controlled substance designated by the board under IC 35-48-2-5 through IC 35-48-2-10 is dispensed, the dispenser is required to submit the following information to INSPECT:
- The patient's name.
- The patient's or the patient representative's identification number.
- The patient's date of birth.
- The national drug code (NDC) number of the controlled substance dispensed.
- The date the controlled substance is dispensed.
- The quantity of the controlled substance dispensed.
- The number of days of supply dispensed.
- The dispenser's United States Drug Enforcement Agency (DEA) registration number.
- The prescriber's United States Drug Enforcement Agency (DEA) registration number.
- An indication as to whether the prescription was transmitted to the pharmacist orally or in writing.
- Other information required by the Board of Pharmacy.
