Anyone 6 months and older may get vaccinated.
Individuals must receive an age-appropriate COVID-19 vaccine. Vaccine types are labeled on the vaccine site map. Sites with COVID-19 pediatric vaccine are marked with a red pin, and the type(s) available are specified in the description. Search the map to find the site closest to you and check the site information for registration details. Some locations require appointments and others allow walk-ins. Call Indiana 211 (866-211-9966) for assistance. Please verify availability in advance by calling the site if possible or contact your healthcare provider.
An adult must accompany children 6 months through 15 years old to the vaccine appointment. If the adult present is not the parent or guardian, consent must be submitted in advance. For anyone age 16-17 it’s preferred that a parent or guardian accompany the minor to the vaccination site. We understand this may not always be possible. In those cases, the parent or guardian can provide written or verbal authorization.
