Mask - You will need to wear it at the clinic to keep you and the person giving you the shot safe.
Proof of eligibility: a) We will need to confirm your date of birth, but you will not need to provide a photo ID for verification, and you can still get the COVID-19 shot if you do not have an ID. Please bring anything that has your name and date of birth printed on it, current or expired. This could be an ID, birth certificate, letter from your doctor, medical record, discharge paperwork, or other document with this information. You will not have to show proof of U.S. citizenship.
Insurance Card (not required) - The COVID-19 shot is free to everyone in Indiana. You do NOT need insurance to get it. However, providers can bill insurance a fee, so bring your insurance card if you have one.
Parent or adult – An adult must accompany children age 12-15 to the vaccine appointment. If the adult present is not the parent or guardian, consent must be submitted in advance. For anyone age 16-17 it’s preferred that a parent or guardian accompany the minor to the vaccination site. We understand this may not always be possible. In those cases, the parent or guardian can provide written or verbal authorization.
