People age 12 and older are eligible to be vaccinated.
Anyone age 12-17 who registers for a COVID-19 vaccine must choose a site that offers the Pfizer vaccine. Vaccine types are labeled on the vaccine site map and indicated on the state’s registration site, too.
An adult, not necessarily a parent or guardian, must accompany children age 12-15 to the vaccine appointment. If the adult present is not the parent or guardian, consent must be submitted prior. For those age 16-17, they will still need consent submitted, but a parent or guardian does not have to be present at the appointment.
