Your agency must submit a letter on agency letter head to the Chairperson of the IDACS Committee:

The letter must state that your agency is requesting to become an IDACS MDD agency. Your Vendor's information and a point of
Contact must be included on the request. The request must be signed by your agency head.
Once a letter of approval to become a MDD agency is received from the IDACS Committee
Chairperson, you will have one year from the date of the letter to actually start the process. Failure to do so will result in the expiration of the approval.
